Lovelorn sanatorium Ⅳ is a Unity-made adult game by TUTGame that builds on the world and play style of Sufferer Rescue Center 3. Development is ongoing. The current beta demo, version 0.01.1, was released on June 25, 2025 for Windows. It is available in English and Chinese and is not censored. The latest changelog marks the demo's initial release.

The game continues the game setting and gameplay of Sufferer Rescue Center 3. The story of the new game tells the story of the protagonist Zhang Ji continuing to search for the key prop mobile phone without the client.
