That Time a Heart Attack Reincarnated Me as the Most Feared Demon Queen of the Dark Continent is a fantasy transformation visual novel by Velvet Weaver, made with the Others engine and 2D plus AI CG art. You play as Gorou, who dies from a heart attack and wakes in the body of a feared Demon Queen. He must survive a royal court, hide his true identity from people who knew the original queen, and manage a kingdom whose subjects are falling into darkness one by one.
The current build is v0.2 Alpha, an ongoing Windows release dated October 15, 2026, and it is uncensored. It supports English, Spanish, Brazilian Portuguese, German, French, Italian, Polish, Russian, Simplified Chinese, Traditional Chinese, Japanese, Korean, Hebrew, Hindi, and Thai. The update is a full remake of v0.1, adding a rewritten prologue with Tiny as a 3D guide, Act I decisions, training with Nemi, a village attack, and a first-person fight against Varrak. It also adds explorable 3D areas, mission tracking, maps, brothel night visits with Alba and Noa, plus minigames and an extra-scene elixir, haggling shop, favours, a hypnosis minigame, photo spots, and inventory systems.

After dying from a heart attack, Gorou awakens inside the body of the most feared Demon Queen on the Dark Continent. Trapped in a dangerously powerful new body, he must survive the royal court, keep his true identity hidden from everyone who knew the real Queen, and somehow hold together a kingdom whose people are going dark one by one.
