The story of The Pink Answer 2 picks up exactly at the end of the first game. You are an agent working for "The Pink Answer," an organization dedicated to taking out the undiserable parts of society and transforming them into beautiful and obidient transwomen to serve and satisfy a very exclusive clientele. Thanks to the resounding success with your first patient, you have been promoted and assigned a new patient. This is a game heavily focused on forced sissyfication, forced feminization, and transformation through the magic of the "Fempils," a drug that will (under very specific circumstances; that's where the player comes in) gradually transform a male into a sissy transwoman.​
