Doctor Simulator is a 2D adult management simulation by CosmicKumo, built in Unity with AI-generated CG visuals. You play a male doctor running a private practice, meeting female patients and persuading them to undress for examinations. The play centers on keeping suspicion low while advancing through clinical interactions, with mature themes including management, humiliation, and male domination.
The current build is v1.18, released May 13, 2025, and development is ongoing. It runs on Windows in English and is uncensored. This update fixes windowed mode so the game can run at many resolutions, adds a story line, and unlocks new intimate scenes for Shreya as relationships improve through shopping district visits. Players can visit Shreya's home to talk, give gifts, change outfits, or engage in sexual actions. Exams now pay money based on how much a patient exposes and their suspicion, with clearer tool-use error messages.

Completing an exam will earn money now, proportional to the maximum level of nudity the patient reaches during the exam as well as patient suspicion
When a tool is used on the wrong body part, the error message "Tool cannot be used on this body part!" is shown
When a tool cannot be used on the character at all, the error message "Tool cannot be used on this character!" is shown
If a tool requires the Secret Basement (eg. syringe), the error message "Tool cannot be used until Secret Basement is owned!" is shown
